This appointment is to complete the required Written Order form for IBHS services. This is NOT a diagnostic appointment, if you need your child to be assessed for an Autism Spectrum Disorder Diagnosis please contact your Primary Care Physician for a referral to diagnostic services.

This is a virtual appointment both you and your child will need to be attendance. If your child is currently receiving services at Success on the Spectrum you can make arrangements with Kevin to have your child attend during the day.
